If you know a child that is interested in joining a hockey team and meets the requirements outlined above, please fill out the form below to apply for a ManAdvantage scholarship. Please contact a ManAdvantage representative at (636)328-1697 if you have any questions.

The following documents must be included in the scholarship application:

  • Most recent school transcripts
  • The most recent tax return
  • Two (2) letters of recommendation
  • A brief narrative of the accident or fatality of the parent/guardian of the applicant
  • For injured parents/guardians: The first report of the injury and the current medical records/rehabilitation reports
  • For parents/guardians who are deceased: A copy of the death certificate

Renewal Application

Recipients of a ManAdvantage Scholarship are invited to apply for renewal consideration until they have graduated high school or reached the age of 18. Please complete your renewal application below. Use your personal statement to summarize your academic and hockey experiences this year, and your goals for next year.

What documentation is needed along with the completed application? The Scholarship Committee will not review your renewal applications until it has received copes of all the documents listed below:

  1. Completed Renewal Application
  2. Completed Coaches Survey
  3. One page personal statement
  4. School Transcript – applicants most recent transcript from school.
  5. Last year’s tax return of parent or guardian.

About the application process

If you or someone you know has a child that has either experienced the death of a parent or has a parent that is incapable of earning an income as a result of their disability (unable to work as defined by the Social Security Act, SSA-3368-BK), that child could be eligible for a ManAdvantage hockey scholarship. We accept applications from both boys and girls under the age of 18. Applicants must be residents of either Missouri or Illinois. No previous involvement with hockey or team sports is required, and scholarships can be applied toward league fees or equipment for ice hockey or roller hockey.

 

Applications will be reviewed by ManAdvantage, and the financial needs of the player as well as the funds available at the time will be assessed. Qualifying applicants can receive a one-season scholarship, which may be contingent upon academic performance. Participants are able to re-apply at the end of the term or school year for which they received the scholarship.

 

Applications and all attached documents are due prior to July 1st of each year.
